how to create radio buttons or checkbox ?

hi all,
ive been trying to use the radio buttons or the checkbox feature but its not even showing off, i cant think whats wrong with the code..im trying to use this sample here :

but its not working something is missing,not sure if its the positon of it or something else...i also searched on the forums for similar issues but the only thing i found is this :
viewtopic.php?f=10&t=1752

can anyone help me with that?

ok i think i found out the way to do it, the issue was prolly related to the buttons position, i thought that it would enherit the positon from its parent just like static texts ...sorry for wasting again ur forums space...and for asking before trying all the alternatives ¬¬...if i have any further doubt about the same subject i will just reuse this thread instead of making another thread for the sake of .db space ..^^
